Documents required by ASL for the supplying of the free prosthesis.
DOCUMENTS REQUIRED
FOR ADULT PATIENTS
1. Medical prescription from an ophthalmologist from a public health structure,
an ASL surgery or Hospital
2. Contact our centre to be sent the estimate of cost.
3.Document of civil invalidity or receipt of the request.
Local GP, CAF, INPS
4.You must deliver the following documents to the prosthesis office of your ASL:

5.RECEIPT OF AUTHORISATION
The authorisation must be handed into our secretaries’ office on the day of the appointment.

FOR PATIENTS UNDER THE AGE OF 18
1. MEDICAL PRESCRIPTION FROM AN OPHTHALMOLOGIST FROM A PUBLIC HEALTH SURGERY OR HOSPITAL
(ASL day surgery or Hospital)
2.CONTACT OUR CENTRE TO BE SENT THE ESTIMATE OF COST
3. DELIVER TO THE PROSTHESIS OFFICE OF THE ASL THE FOLLOWING DOCUMENTS:

4.RECEIPT OF AUTHORISATION
The authorisation must be handed into our secretaries’ office on the day of the appointment.
Documents required for receipt of the prosthesis for patients assisted by INAIL:
For patients assisted by INAIL on all the national territory, it is sufficient to present the estimated cost from our secretaries’ office directly to your local INAIL office.
You will be contacted by us as soon we have received the authorisation directly from your INAIL branch.
